Director of Social Services Position Type:
Full-Time 40hours Responsibilities of Director of Social Services:
Identifies cognitive impairments, mood problems, and other psycho-social needs and follows-up as needed.
Schedules and takes the lead in interdisciplinary meetings with residents and their families.
Maintains accurate, thorough, and timely documentation.
Reviews Advanced Directives and provides education to residents and families.
Acts as a resident advocate and is familiar with residents' rights.
Makes appropriate referrals to outside agencies.
Helps provide a smooth admission and discharge process for residents and families.
Acts as a liaison for ancillary services.
Qualifications of Director of Social Services:
Must have a Bachelor of Social Work.
Must have an active Social Work license in good standing.
Experience in a nursing facility required.
